For some reason, whenever I push an update to my site using Envoyer/Forge (which just grabs the most recent VCS commit from GitHub), the Media Manager disappears & the updates page rolls back to an older version like this:

Checking for updates and then doing a "force update" fixes it, but it happens every single time and I'm trying to find the underlying problem. My local version of the site has the media manager there and the updated updates page, so I don't see why when the commit is cloned on the VPS it rolls back and the media manager disappears?

It sounds like it is not grabbing the most recent VCS commit from GitHub as you suggest.

This has been happening to my project ever since the Media Manager has been added though and since then, I have made easily 100+ commits. The site has definitely been grabbing the most recent commits because the rest of the site is getting properly updated each time. Everything is updating and working completely fine except for this one strange thing so I'm really confused.
