if you create a new form in backend you also need to create a new form template for the component

for example form "contact_form" has plugins/ozc/contact/components/contact/contact_form.htm so your new form "enter_club" needs a plugins/ozc/contact/components/contact/enter_club.htm

of course its better to copy the component templates into your /theme/--your-theme--/partials folder

Go into Backend >> ocz Contact >> Settings and open Tab "Form Config". Click on the Settings for you Form and open the Tab "error messages". Input a Error Message for each form Validation.
