Did you log out and back into the admin / backend interface? It sounds like the install did not complete and migration creation of tables did not take place.

That did the trick. I did not realize a logout was required to process a migration.

Is there anyway to force a migration from the CLI?

php artisan plugin:refresh Author:Plugin

Caution: This will flush plugin tables too!
